We require some parental sign-offs prior to admission of students to the school.  Please click on the following to access the sign-off form:  Click here for the Parent Permissions form

The following forms are also required to begin the admission process. The forms are currently only available via email from our Registrar, Cherine Fayad at cherine.fayad@kaust.edu.sa

Medical Form

A fully completed medical form signed by parents or legal guardians and the examining physician, is required prior to the student’s start date. Please see the medical section for a vaccination schedule.

Education or Emotional Support Documentation

These documents (if applicable) include, but are not limited to, copies of all school reports, copies of all diagnostic test reports and results and the student’s most recent IEP.

Student Information Form

This form must be completed for all students and submitted directly to the Office of Admission.

Enrollment Agreement

This form is a signed agreement by a parent or a legal guardian and should be returned to the School prior to the student's start date.

School Bus Transport Registration Form

(If your child(ren) requires bus service) This form must be completed, signed and returned to the School Office at least (5) business days prior to the start of the transportation service.
